Initial Assessment of needs and your individualized plan of action: collecting medical history, activities of daily living (mobility, exercise, social habits, lifestyle routines) medications/dosages, primary physician contact.

Once the initial assessment is completed you have an established partnership with us. We are now your advocate and can be available to you if any further health issues arise or that you cannot advocate for yourself or your loved one.

Our Primary Services

Medical/Navigational assistance: coordinate care among physicians and other health care providers that are treating you.

Background Research: Diagnosis, treatment options, second opinion options. (Information regarding possible options will be documented and given to client/patient and family)

Hospital bedside or travel accompaniment to appointments: Aid in preparing of questions to ask health care providers. Extensive documentation of all encounters with medical providers (copies to client/patient or other designated individual) (per hour, free travel up to 50 miles from Morehead City)

Prevention: prescription drug review (maintain a current list of medications while considering medication options to reduce risk of error), health/wellness coaching, weight loss, Immunities, etc.

End of life planning: living will, health care power of attorney, advance directives, palliative care, hospice

For those clients that have complex health conditions and would like more managed services we have a membership plan. By Being a member, it will allow you to take control of your health, whether your needs are large or small. We can be an extension of your family, a resource for you and help minimize the stress if anything acute should happen; we can even be your emergency contact in your time of need if your family lives away from you if you are an established member, giving you the peace of mind you need.

Primary Plan of Action

For healthy clients who need someone to talk to about their health or medication concerns periodically, or if you live far away from your loved one and want a professional Advocate to check in on them on a consistent basis.

Secondary Plan of Action

This plan works best for someone who may have 1 or 2 chronic conditions, but their health is relatively stable, or someone who needs a little more support every week to be sure they are meeting all their health needs.

Tertiary Plan of Action

A new diagnosis, recent hospital discharge or worsening chronic conditions brings a lot of stress and unknown. You will need more time with an advocate to help you through these acute phases.

Quaternary Plan of Action

When you need help finding the resources continue living at your highest quality of life. We can help you find the resources to remain living at home, find the right facility for you, or provide significant support for a health problem.
