We provide an initial assessment of needs: collecting medical history, activities of daily living (mobility, exercise, social habits, lifestyle routines) medications/dosages, and primary physician contact. This will then result in your initial Plan of Action.
Once you have an established partnership with us. We are now your advocate and can be available to you if any further health issues arise or if you cannot advocate for yourself or your loved one.
For those clients that have complex health conditions and would like more managed services, we have a membership plan. Being a member will allow you to take control of your health, whether your needs are large or small.
We can be an extension of your family, a resource for you, and help minimize the stress if anything acute should happen; we can even be your emergency contact in your time of need if your family lives away from you or if you are an established member, giving you the peace of mind you need.
